Signing up for the first time.​

For patients already registered at our clinic, if you are booking/signing up for the first time, please click "Log In" at the top right of this page, and Sign Up by submitting your FULL NAME as it appears on your Care Card. Once we locate your patient chart and confirm that you are a patient registered with either Dr. Lew or Dr. Allaye-Chan, your account will be activated. It may take up to 24 hours to receive a reply to your sign-up application.
Canceling appointment bookings on an Android device.​

It has been brought to our attention that when accessing our online booking page using an Android device, some patients have had difficult rescheduling or canceling booked appointments. In this case, please download the Wix app from your App Store. Then, review the site membership confirmation email you received when signing up, and click the "Join" button. Once the Wix App opens, log in your with email and password. Once logged in, you can make changes to any booked appointments.
I've previously signed up for the online booking service and used it to book an appointment to see the doctor, but now I can no longer access the Book Online page!

We rely on the FULL NAME provided to confirm that you are a patient of our clinic. Please update your account with this information, THEN call our office. We will take down your name, and approve your account if appropriate. Once approved, you will be able to visit the Book Online page and book future appointments. In the meantime, if your name is already registered as a patient of one of our doctors, and you require an urgent appointment, we can assist you in booking an appointment via telephone. I'd prefer to continue booking my appointments via telephone!

If you'd prefer to book your appointments via telephone, you may do so. However, please know that our clinic schedule is arranged to favour appointments booked via our online booking service. There are more time slots available when booking online. Patients booking by telephone or in person at the front desk choose from only a limited amount of "telephone appointment" time slots.
